Ryan Erwin on optimizing an enterprise imaging cloud transformation

Accessium Group founder and CEO Ryan Erwin wrote a guest article for Fujifilm’s EI Connection blog on what a health system should settle before it moves enterprise imaging to the cloud.

The article, Four considerations to help optimize your enterprise imaging cloud transformation, was published by FUJIFILM Healthcare Americas Corporation. It closes by pointing readers to the main-stage session Ryan gave at HIMSS 2025 two months earlier, How to optimize your PACS and VNA cloud transformation, which Fujifilm sponsored. That session is the video above, and it runs about 24 minutes.

The four considerations

• Data migration. Catalogue every clinical workflow, application and capability before anything moves. Large imaging datasets are their own problem, which is what AWS Direct Connect and AWS Snowball exist for, and some enterprise imaging applications carry tools that accelerate the transition and place data on cheaper storage tiers as it ages.
• Security and compliance readiness. A cloud provider’s shared responsibility model covers the provider’s half of the estate. HIPAA, HITRUST and SOC 2 obligations stay with the health system, and a zero-trust identity layer belongs in the design rather than after it.
• Staff training and process shifts. Most skills transfer. Portals, terminology and daily operational habits do not, so a team that has run an on-premises estate for a decade needs formal training to run the same estate in the cloud.
• Managing cloud costs effectively. The move is from capital expenditure to operating expenditure, which changes who approves spend and how often. Model the workloads with something like AWS Cost Explorer before the migration, rather than reading the first invoice as a surprise.

Cloud transformation for imaging is not a general IT migration with pictures in it. The datasets are large enough that the transfer method is a design decision, the applications are clinical, and the compliance obligations do not move to the provider along with the workload. That is the thread through both the talk and the article.
